Service Invoice

This one is different from your traditional invoice. You will see on the left three boxes where the company and shipping address are listed. To its right we have to enter the service number, quantity purchased, service description, unit cost, and total cost respectively. The subtotal is adjusted with applicable fees/taxes and the total amount is made know to the customer.
